Possible Additional Charges

Cosmetic mole removals are an additional $150 per mole on the face/scalp and $75 per mole on the body. Cyst removals are a minimum $100 additional per cyst and exact pricing will vary depending on size/location/complexity.

The doctor may send removed moles/cysts and other skin samples to be tested at specialized facilities, order labs and prescribe pharmacy-filled prescriptions. Most local insurance plans cover these third-party services, but it may generate additional charges due to deductibles and copays. Some HMO plans (eg Kaiser), Tricare plans and Quest plans often will not cover third party services without a preauthorization or referral from a primary care doctor. For these patients and non-insured patients, we have pre-negotiated discounts and coupons available.

Other skin surgery, Botox, dermal fillers / lip injections, microneedling, medium/deep chemical peels, in-office dispensed prescriptions and skin care products incur additional charges, which will be discussed with you prior to rendering.

Insurance, Reimbursement, FSA/HSA

Honolulu Dermatology does not accept insurance and is out-of-network with all insurance plans. However, most local PPO-type plans reimburse for medically necessary services provided by out-of-network doctors. Many of our patients receive a reimbursement of $100-$150 for their initial appointment and $80-$120 for a follow-up appointment if they have already met their annual deductible.

If you have one of the following plans we will prepare a reimbursement claim form for you to submit upon request: HMSA PPO, UHA PPO (600/3000), Mainland Blue Cross or Blue Shield PPO/POS, BC/BS Federal Employee Program PPO, or AFL Hotel & Restaurant Workers Trust Fund PPO. In order to receive a reimbursement claim form, you must fill out the insurance information section of the intake form.

Other insurance plan types do not reimburse for services provided by out-of-network doctors such as HMO plans (eg Kaiser), Medicare (including Advantage plans), Medicaid (Aloha Care and Quest plans), and Tricare plans.

For possible reimbursement from HMAA/HWMG and other mainland PPO and POS plans, as well as itemized receipts for FSA/HSA/HRA, please request a detailed receipt following your appointment.
